Brooks Rehabilitation

The Brooks San Jose Outpatient Facility is conveniently located inside the JCA. We provide traditional physical therapy as well as aquatic therapy. Typical diagnoses seen are low back pain, neck pain, elbow and shoulder injuries, knee pain (pre- and post-operative knees included), hip pain (pre- and post-operative), ankle and foot injuries. 

If you are interested in receiving physical therapy for an injury or chronic condition, please speak with your primary care physician about obtaining a referral.

Hours:
Monday 8 a.m. - 7 p.m.
Tuesday 8 a.m. - 6 p.m.
Wednesday 8 a.m. - 4 p.m.
Thursday 8 a.m. - 7 p.m.
Friday  8 a.m. - 4 p.m.

(Closed for lunch 1 - 2 p.m. each day)
